Qualifying Medical Conditions in Illinois

Submit your application:
- After seeing our cannabis-friendly doctor, we will submit your application free of charge. Within 48 hours, you will be able to go to the dispensary of your choosing.
Terminal Illness Patients
Patients that have been diagnosed with a terminal illness with a probable life expectancy of six (6) months or less may apply for a for six month card. This card only requires one visit and there is no state fee.
Minors and Parents/Guardians
If you have considered medical cannabis/cannabidiol to help your child's medical condition, you have found the right place. Minor patients may only purchase medical cannabis-infused products from medical cannabis dispensaries. Minor patients are allowed to have a caregiver. Caregivers can enter their designated dispensary and purchase medical cannabis-infused products.
