Qualifying Medical Conditions in Illinois
All adult patients 18 years old and over must be diagnosed with one of the following conditions and establish a relationship with the signing doctor. Any adult patient who needs a caregiver can add one. Medical cannabis cards are valid for one, two or three years.

Qualifying Medical Conditions in Iowa
All adult patients 18 years old and over must be diagnosed with one of the following conditions and establish a relationship with the signing doctor. Any adult patient who needs a caregiver can add one. Medical cannabis cards are valid for one year and must be renewed.
- AIDS or HIV
- Amyotrophic lateral sclerosis (ALS)
- Autism (self-injurious or aggressive behaviors)
- Cancer
- Chronic pain
- Corticobasal Degeneration
- Crohn's disease
- Multiple sclerosis
- Parkinson's disease
- Post-Traumatic Stress Disorder (PTSD)
- Seizures
- Any terminal illness with a probable life expectancy of under one year
- Ulcerative colitis
Terminal Illness Patients
Illinois residents:
Patients that have been diagnosed with a terminal illness with a probable life expectancy of six (6) months or less may apply for a for six month card. This card only requires one visit and there is no state fee.
Iowa residents:
Patients that have been diagnosed with a terminal illness with a probable life expectancy of 1 year or less may apply.
Minors and parents/ guardians
If you have considered medical cannabis/cannabidiol to help your child's medical condition, you have found the right place. Minor patients are allowed to have a caregiver. Caregivers can enter their designated dispensary and purchase medical cannabis-infused products.
